Update your registration details online

Use the secure MedLink form to tell us about changes to the personal and contact information held on your GP record. Keeping these details current helps us contact you safely and prevents confidential messages being sent to an old number, email address or address.

The form opens on the MedLink website in a new tab. MedLink processes the information so it can be sent securely to the practice.

When to use this form

Use it when registration information changes, such as your:

  • home address or postcode;
  • mobile or telephone number;
  • email address;
  • preferred communication details;
  • other personal details requested on the form.

If you have moved outside the practice area, submit the new address accurately. The practice will review it and contact you if your registration may be affected.

What happens after you submit it?

  1. We match the form to your record

    Reception checks that the information relates to the correct patient.

  2. We review the changes

    We may contact you if information is incomplete, conflicts with the existing record or needs supporting evidence.

  3. We update the GP record

    Approved changes are entered into the practice's clinical system. Relevant contact and online-service settings are reviewed where needed.

Turning 16

Patients approaching their 16th birthday may be asked to use this form to confirm their own contact details. This helps protect confidentiality and supports the move to the patient's own online-services arrangements.

  • repeat prescription requests;
  • appointment requests or cancellations;
  • complaints or sensitive clinical correspondence.
