Request an additional sick note
If your illness continues beyond your current fit note, you can ask for an extension without booking an appointment.
This page is about extending an existing fit note only. If you have been off sick for 7 calendar days or less, you can self-certify using form SC2 from GOV.UK or your employer. If you need a first fit note, an appropriate assessment by a clinician is required — see Sick and fit notes.
Request an extension online
We no longer use an online form for this request. The quickest way to ask for an extension is through Patient Services — the same website you use to order repeat prescriptions. Sign in and, if an administrative request option is shown, send a request telling us:
- The date your current fit note runs out
- The condition stated on your current fit note
- Whether your condition has changed since your last note — and if so, briefly what has changed
Your request will be passed to a clinician. We will contact you if a consultation or further information is needed. Please request an extension in good time, but do not worry if your fit note has recently expired — fit notes can usually be backdated where clinically appropriate.
Online functions vary by account. If the option is not shown, or you cannot use Patient Services, phone 01968 675 576 between 8:30am and 5:30pm, Monday to Friday. Do not send the request under an unrelated category. Not yet signed up? See Register for online services.
Do not use Patient Services for urgent medical problems. In an emergency call 999. When the practice is closed and you need urgent advice, call NHS 24 on 111.
